From entry to exit: Navigating the highs and lows of transitioning in and out of the Care System

Entering and leaving the care system are two of the most critical transition points in a young person’s life - often filled with uncertainty, anxiety and trauma.
This online session will explore the challenges care-experienced young people face at these key stages, including the loss of stability, lack of control, identity struggles, and the long-term effects of poorly managed transitions.

About the session

Through a mix of lived experience insights, professional reflections, interactive discussions, and practical tools, we’ll examine:
  • The emotional and psychological impact of entering care
  • Gaps in current leaving care support systems
  • Culturally responsive approaches and trauma-informed practices
  • What practitioners, carers, and systems can do better
This session is ideal for youth workers, social workers, educators, and anyone passionate about improving outcomes for care-experienced young people. Together, we’ll unpack what it really means to create safety, continuity, and belonging during the most vulnerable moments in a young person’s care journey.

Jade Barnett

Jade Barnett is the Director & CEO of Power2Prevail Community, a dynamic public speaker, consultant, and facilitator.

Born and raised in South East London, Jade’s journey has been shaped by her lived experience - permanently excluded from school and entering the care system at 15. These early challenges became the foundation of her mission to advocate for vulnerable and marginalised young people, using her voice to influence systems and shift narratives.

Since 2020, Jade has championed better outcomes for young people facing adversity. She has featured in national campaigns including Nothing in Isolation (Mission 44 & Sir Lewis Hamilton) and Gone Too Far (Become Charity), and was a finalist for the Young Entrepreneur Award at the 2024 Precious Awards. From 2021–2023, she served on the Mayor of London’s Violence Reduction Unit’s Young People’s Action Group.

Jade is widely recognised for her advocacy in transforming the social care system by championing cultural competency, representation, relatability, and youth voice. Her flagship social action event, LOVE ‘OUR’ YOUTH, raised awareness of the lack of youth provisions in South London, while her self-designed Journey2Adulthood programme, delivered via her organisation Power2Prevail Community provides vital support for care-experienced young people aged 16–25 as they prepare for their transition into independence.
